Description

Arising from the anteromedial orbital floor, the inferior oblique muscle courses posterolaterally beneath the globe, passing inferior to the inferior rectus before spreading toward the posterolateral sclera deep to the lateral rectus. Around it, the four rectus muscles form a muscular cone around the eyeball, with the medial rectus on the nasal side and the lateral rectus on the temporal side. Posteriorly, the optic nerve exits the globe at the optic disc and runs back through the orbit toward the optic canal, while adjacent orbital bone defines the confines of the orbital cavity. Veins on the scleral surface reinforce the anterior orientation of the globe. Inferior oblique anatomy matters because its line of pull is easy to misunderstand in isolation, and this view makes its relationship to the inferior rectus and lateral globe unambiguous. When the eye is adducted, the inferior oblique acts as the primary elevator and also contributes to abduction and extorsion, a pattern used every day when interpreting extraocular motility exams and Bielschowsky head-tilt testing. Inferior oblique overaction, common in congenital strabismus and in association with superior oblique palsy, is treated with procedures such as inferior oblique recession or myectomy, where recognizing its origin on the orbital floor and its proximity to the inferior rectus reduces iatrogenic restriction. Use this rendering in ophthalmology and neuroanatomy teaching to explain extrinsic (extraocular) muscle actions, oculomotor nerve (CN III) targets, and the geometry of the muscle cone around the optic nerve. It also fits well in surgical atlases and patient-education materials for strabismus correction, where a clean 3D overview beats a flat cross section for explaining spatial relationships.
