Description
Anteriorly oriented, frontal cross-section anatomy of the adult female urinary bladder centers on the detrusor muscle (musculus detrusor vesicae) with emphasis on an unstratified region (pars nonstratificata). The muscular wall encircles the vesical lumen, with the mucosa and submucosa positioned internally and the adventitia or serosa lying externally, depending on bladder surface. Superiorly, the dome (apex) curves toward the peritoneal reflection, while inferiorly the base (fundus) approaches the bladder neck and proximal urethra. Attention to a non-stratified detrusor segment matters when teaching how the bladder generates coordinated pressure during filling and voiding without discrete, easily separated muscle layers, a point that can be obscured in schematic “three-layer” depictions. Surgeons working near the anterior bladder wall, including retropubic approaches for stress urinary incontinence (for example, Burch colposuspension or mesh sling passage), benefit from a clear mental map of the detrusor thickness and its relationship to the lumen to reduce risk of cystotomy.
