Description
Anteriorly, the pulmonary trunk (truncus pulmonalis) rises from the right ventricle and bifurcates into the right and left pulmonary arteries, with the left pulmonary artery (arteria pulmonalis sinistra) coursing laterally toward the left lung hilum. At the base of the heart, the ascending aorta sits posterior and slightly to the right of the pulmonary trunk, while the aortic arch begins superiorly and sweeps leftward above the pulmonary artery bifurcation. From this frontal orientation, the left pulmonary artery is appreciated as the leftward branch of the pulmonary trunk, positioned anterior to the left main bronchus as it approaches the root of the lung. This angle matters when teaching the normal relationships that underpin congenital and acquired pathology at the great vessel level. Patent ductus arteriosus connects the proximal left pulmonary artery to the inferior surface of the aortic arch near the aortic isthmus, and a ductal aneurysm or persistent shunt changes the caliber and contour of the left pulmonary artery on angiography and CT. Surgeons and interventionalists also use these landmarks during pulmonary artery catheterization and during repair of conotruncal defects, where malposition of the great arteries (for example, transposition of the great arteries) reframes what is anterior and what is posterior at the arterial roots. Small shifts here have large consequences. Use this illustration in cardiothoracic anatomy and embryology teaching to anchor discussions of truncus arteriosus septation, pulmonary trunk bifurcation, and the ductus arteriosus remnant (ligamentum arteriosum) in the adult.
