Description
Anteriorly, the left lateral division of the male liver is presented as the parenchyma lateral to the falciform ligament and left hepatic fissure, corresponding to Couinaud Segments II (superior) and III (inferior). Segment II occupies the more cranial and posterolateral portion of the left hepatic lobe, while Segment III sits inferiorly and more anteriorly toward the left margin. The inferior border forms a thin, sharp edge, and the left triangular ligament attachment would be expected near the superolateral limit, where the diaphragmatic surface turns toward the posterior aspect. Segmental separation is implied by the course of the left hepatic vein draining superiorly toward the inferior vena cava, even if deep venous landmarks are not fully exposed in a purely anterior view. Isolating the left lateral division matters because it is the anatomic target for a left lateral sectionectomy, a common parenchyma-sparing hepatic resection used for colorectal liver metastases, hepatocellular carcinoma in cirrhotic patients, and living-donor transplantation of the left lateral segment for pediatric recipients. Segment II and III anatomy also underpins safe laparoscopic approaches, where the proximity of the umbilical fissure, falciform ligament, and left hepatic vein guides inflow and outflow control. Small differences in how the left lobe drapes anteriorly can change the working window for stapling or energy-based parenchymal transection. Landmarks matter.
