Description
Centered in the superior mediastinum, the thoracic trachea descends from the thoracic inlet toward the carina, maintaining a midline course anterior to the esophagus and posterior to the manubrium and great vessels. C-shaped tracheal cartilages form the anterior and lateral walls, while the posterior wall remains membranous, bridged by the trachealis muscle. The lumen sits immediately anterior to the prevertebral fascia, and the tracheoesophageal groove lies posterolaterally on each side. Attention to the posterior membranous wall matters when teaching dynamic airway caliber and when planning airway instrumentation, because this segment can bow posteriorly with coughing or positive pressure ventilation and is the site most susceptible to iatrogenic injury. Tracheomalacia often presents as excessive expiratory collapse related to weakened cartilaginous support, whereas fistulization into the esophagus can follow prolonged cuff pressure, producing aspiration and recurrent pneumonia. Anatomy drives the procedure. Common use cases include thoracic surgery and anesthesiology teaching modules on tracheal intubation, tracheostomy complications, and mediastinal relationships, as well as radiology atlases correlating bronchoscopy and axial CT anatomy at the level of the thoracic trachea and carina.
