Description
Posteriorly oriented within the superior mediastinum, the thoracic part of the trachea (pars thoracica tracheae) is presented with its C shaped hyaline cartilage rings opening dorsally into the flattened musculomembranous wall. Trachealis muscle fibers bridge the posterior ends of the cartilages, creating a compliant posterior boundary that lies immediately anterior to the esophagus. Inferiorly the airway continues toward the carina, while laterally the trachea relates to mediastinal pleura and the proximal course of the main bronchi. This posterior relationship is the key teaching point. Esophageal distension during swallowing can indent the trachea across the trachealis, and the same interface becomes clinically relevant during bronchoscopy and airway management, where posterior wall laxity, iatrogenic injury, or overinflation of an endotracheal tube cuff can compromise the membranous trachea. Surgeons also use these planes during tracheal mobilization and repair, where recognizing the noncartilaginous posterior wall helps avoid unrecognized tracheoesophageal fistula and guides suture placement in tracheal lacerations.
