Description
Anterior aspect of the left lung is presented with the bronchopulmonary anterior segment (segmentum anterius) distinguished by color within the upper lobe. Medially, the costomediastinal surface approaches the hilum, while the anterior border courses inferiorly toward the cardiac notch and lingula. Superior and posterior to the highlighted segment lie the apicoposterior portions of the left upper lobe, with the oblique fissure trending inferoposteriorly to separate upper and lower lobes. The diaphragmatic surface sits inferiorly, out of full view in a strict frontal perspective. Clinically, isolating the anterior segment matters because endobronchial anatomy and segmental ventilation patterns often dictate where infection, aspiration, or atelectasis localizes, and which bronchus a bronchoscopist targets for lavage or biopsy. Segmentectomy planning in early-stage non-small cell lung cancer depends on accurate segment identification and appreciation of the intersegmental plane, which is often defined intraoperatively with inflation deflation techniques or indocyanine green perfusion. Keep the left sided nuance in mind: the upper lobe anatomy includes the lingular segments, and the anterior segment can be mistaken for lingular involvement on plain radiography when opacities project along the left heart border. Orientation is everything.
