Description
Framed within an anterior cross section of the right atrium, the fossa ovalis sits on the interatrial septum as an oval depression, bordered superiorly and anteriorly by the limbus fossae ovalis (annulus ovalis), a raised muscular rim. Medial to the atrial free wall, this margin defines the transition between the thinner floor of the fossa (valve remnant of the foramen ovale) and the thicker septal tissue surrounding it. The limbus arcs around the fossa like a ridge, giving a clear anatomic border for orientation within the atrial chamber. Attention to the limbus matters because it is the consistent landmark used to localize the fossa ovalis during transseptal puncture for left atrial access in procedures such as catheter ablation for atrial fibrillation, mitral valve interventions, and left atrial appendage closure. Puncturing too anteriorly risks the aortic root; too posteriorly increases the chance of pericardial entry and tamponade, so operators aim for the thin fossa rather than the thicker limbus. A common teaching point also sits here: a patent foramen ovale reflects incomplete postnatal fusion in this region, and the relation of flap valve to the limbus helps explain right to left shunting during transient increases in right atrial pressure. Cardiology and cardiac anatomy courses can use this view to teach embryologic remnants of the foramen ovale and the practical landmarks of the interatrial septum.
