Description
Sectioned through a renal pyramid, the view centers on the area cribrosa at the papilla renalis, where multiple foramina papillaria perforate the apex and open into the minor calyx. Converging collecting ducts (ductus papillares, ducts of Bellini) approach the papillary tip from proximal to distal within the medulla, while the surrounding medullary tissue tapers inferiorly toward the calyceal space. Anterior and posterior relationships are oriented to the section plane, with the papillary surface forming the most distal medullary landmark. Clinically, the cribriform area is where urine leaves the kidney parenchyma and enters the collecting system, so it anchors teaching on the transition from collecting ducts to calyces and renal pelvis. Papillary necrosis, seen with diabetes mellitus, analgesic nephropathy, or sickle cell disease, targets this region and can lead to sloughed papillae obstructing the calyx with hematuria and colicky pain. Endoscopic and imaging correlations also land here: the papillary tip and minor calyx are key reference points during ureteroscopic evaluation of upper-tract bleeding sources and when localizing sites of intrapapillary calculi in medullary sponge kidney.
