Centered within the renal medulla, a single renal pyramid is presented in cross-section, with its apex forming the renal papilla and its broader base directed superiorly toward the corticomedullary junction. The papilla projects anteriorly toward the renal sinus, where it would abut a minor calyx, while medullary tissue fans laterally on either side of the cone-shaped pyramid. Near the papillary tip, the collecting ducts converge into papillary ducts (ducts of Bellini) that open at the area cribrosa on the papillary surface. This cut emphasizes the terminal pathway for urine as it exits the collecting system of the medulla and enters the calyceal system, a relationship that underpins papillary necrosis and reflux-related scarring. Diabetes mellitus, analgesic nephropathy, and sickle cell disease preferentially injure the papilla because the inner medulla runs at the edge of oxygen delivery and depends on a delicate vasa recta supply. Small anatomy, big consequences. Educators use this view to teach medullary architecture, corticomedullary organization, and the functional funneling of urine from collecting ducts to minor calyces in renal physiology and histology blocks. It also supports urology and radiology discussions of sloughed papillae, filling defects at the calyces on urography, and endoscopic navigation near the papillary tip during ureteroscopy.
