Description
Anteriorly oriented, the heart is presented with emphasis on the apex cordis, the inferolateral tip formed predominantly by the left ventricle. The apex lies inferior to the base of the heart and left of the midline, with the right ventricle occupying more of the anterior surface superior and medial to it. A clear pericardial outline frames the organ while adjacent vasculature is intentionally omitted, so the contour of the ventricular myocardium and the general position of the anterior interventricular sulcus read cleanly. No vessels. Just myocardium and form. Apex-focused teaching images matter because the point of maximal impulse (PMI) is clinically anchored to this anatomy, classically palpated in the left 5th intercostal space at or near the midclavicular line when cardiac size and position are normal. Left ventricular hypertrophy, cardiomegaly, or a large pericardial effusion can displace or blunt that impulse, while hyperinflated lungs in COPD can reduce palpability and subtly alter perceived location. Cardiac surgeons and echocardiographers also reference the apex when aligning apical windows and when describing apical aneurysm or thrombus after anterior myocardial infarction.
