Description
Dominating the anterior (sternocostal) surface, the right ventricle forms most of the visible myocardium, while the left ventricle contributes the left border and converges inferiorly into the cardiac apex. Superiorly, the right and left atrial auricles project like ear-shaped appendages, flanking the proximal outflow region but without the great vessels present to obscure the atrioventricular junction. A shallow anterior interventricular sulcus marks the plane between ventricles as it runs toward the apex. No vascular stumps interrupt the base. Stripping away the aorta and pulmonary trunk is more than a stylistic choice, it clarifies the surfaces surgeons and trainees often struggle to orient when the pericardium is opened and the heart is mobilized. This anterior presentation makes it easier to teach why the right ventricle, not the left, is the chamber most exposed behind the sternum and why penetrating anterior chest trauma tends to involve right ventricular free wall injury and hemopericardium. It also supports discussion of the pericardiocentesis window, where needle trajectories are planned with the cardiac silhouette and apex position in mind rather than the branching pattern of the great vessels.
