Description
Dominating the anterior (sternocostal) surface, the right ventricle lies inferior to the right atrium while the left ventricle forms the left (obtuse) margin and apex. Running obliquely toward the apex, the anterior interventricular sulcus separates the ventricles and typically carries the anterior interventricular artery (left anterior descending, LAD) accompanied by the great cardiac vein. Superiorly, the ascending aorta emerges centrally with the pulmonary trunk positioned anterior and slightly leftward, and the right atrial appendage sits anterosuperior on the right as the left auricle overlaps the proximal pulmonary trunk. For teaching coronary topography, this angle is hard to beat because it ties external landmarks to the arterial territories clinicians discuss daily: LAD across the anterior interventricular sulcus, right coronary artery in the right atrioventricular (coronary) sulcus, and circumflex artery tracking leftward in the left atrioventricular groove. When you are correlating a classic anterior STEMI pattern (V1 to V4) with LAD occlusion, the visible course to the apex and its relationship to the left ventricular anterior wall makes the surface anatomy immediately actionable, including why proximal lesions can endanger septal perforators and conduction pathways.
