Description
Anteriorly, the cardiac apex (apex cordis) is presented with the apical notch (incisura apicis cordis) emphasized at the most inferior point of the ventricular mass. The left ventricle forms the dominant contour, while the right ventricle lies more superior and medial in the foreground of the sternocostal surface. Superior to the notch, the anterior interventricular sulcus is implied by the change in curvature where the ventricles meet, but the usual vascular companions are absent. No vessels are shown. For teaching topographic anatomy, this isolated notch matters because clinicians palpate the apical impulse near the left 5th intercostal space at the midclavicular line, and the ventricular geometry that produces the apex changes with disease. Left ventricular hypertrophy, dilated cardiomyopathy, and large pericardial effusions can displace or dampen the point of maximal impulse, while true dextrocardia reverses expected laterality, all of which are easier to explain when the apex is not visually competing with the left anterior descending artery or great cardiac vein. It also supports discussions of why apical thrombus formation follows anterior myocardial infarction, even when coronary anatomy is taught separately. Faculty can drop this plate into gross anatomy and cardiothoracic modules to anchor terminology (apex cordis, incisura) before introducing coronary surface anatomy, and publishers can pair it with physical exam figures on precordial landmarks and apical impulse assessment.
