Description
Cut through a typical renal (Malpighian) pyramid at its broad base, the section meets the inner margin of the renal cortex and opens onto the corticomedullary junction. Collecting ducts and straight segments of nephrons (tubuli recti) course in parallel within the medullary tissue, while medullary rays from the cortex align with the pyramid’s axis toward the papilla renis. Arcuate vessels lie superior to this boundary along the base of the pyramid, giving rise to interlobular branches that run into the cortex as the pyramid tapers distally toward its papillary tip. That corticomedullary interface is a key teaching landmark because it is where cortical pathology and medullary pathology start to separate anatomically, and it is also where vascular anatomy changes from arcuate to interlobular patterns. Clinically, the arrangement of straight tubules and collecting ducts at the pyramid base underpins the medullary osmotic gradient, a concept needed to understand concentrating defects seen with loop diuretics and medullary ischemia.
