Description
Centered on the trigone of the adult female urinary bladder, the ureteric orifice is shown as it pierces the posterolateral bladder wall and opens onto the mucosal surface. The intramural (submucosal) course of the ureter runs obliquely from superior and lateral toward inferior and medial before reaching the lumen, creating the characteristic slitlike appearance that helps resist reflux. Medial and slightly inferior to each ureteral opening, the interureteric ridge (Mercier bar) spans the trigone toward the contralateral side, with the bladder neck lying inferior in the midline. That oblique ureterovesical junction is where anatomy and function meet, and small distortions have outsized consequences. Shortening of the intramural ureter can predispose to vesicoureteral reflux, while edema, impacted distal ureteral calculi, or iatrogenic injury during pelvic surgery can obstruct the ureteric orifice and produce upstream hydroureteronephrosis. During cystoscopy, the ureteral opening and trigone serve as fixed landmarks for ureteral catheterization, stent placement, and for recognizing the typical location of ureteroceles or papillary urothelial tumors near the orifices.
