Description
Positioned on the posteroinferior wall of the adult female urinary bladder, the trigone forms a smooth triangular field bounded superolaterally by the right and left ureteric orifices and inferiorly by the internal urethral orifice. Each ureter enters the bladder obliquely, its intramural segment coursing through the detrusor before opening as a ureteral opening at the trigone, a configuration that lies posterior to the bladder neck and immediately superior to the proximal urethra. The interureteric ridge (Mercier’s bar) connects the two ureteric orifices as a shallow transverse elevation. Small, constant landmarks. Understanding the ureteric opening in the trigone matters because this region is fixed relative to the rest of the bladder and anchors endoscopic orientation during cystoscopy. The oblique intravesical ureteral tunnel creates the antireflux mechanism; when that tunnel is congenitally short or disrupted, vesicoureteral reflux and recurrent pyelonephritis become real clinical outcomes.
