Description
Centered on the concave medial border (margo medialis) of the reniform kidney, the renal hilum (hilum renale) sits near the midportion of the organ as the focal interruption of the smooth capsular contour. The renal pelvis and proximal ureter emerge from the hilum, continuous with the major calyces deep to it, while the renal vein lies anterior to the renal artery at the entry point into the renal sinus. Superiorly the upper pole approaches the suprarenal (adrenal) gland, and the hilum area is framed by the anterior and posterior lips of the renal parenchyma that form the medial cleft. Orientation matters. A true medial view clarifies the pathway from collecting system to ureter and the relationship of the hilar vessels, the practical map for open or laparoscopic nephrectomy and for hilar dissection during partial nephrectomy. Surgeons plan around the anterior-to-posterior sequence (vein, artery, pelvis) and around common anatomic variation such as accessory renal arteries entering near the hilum or directly into the poles, a frequent source of segmental ischemia if missed. This is also the perspective that aligns well with CT urography correlations when tracing hydronephrosis back to a ureteropelvic junction obstruction.
