Description
Posterior to the liver, the porta hepatis occupies the transverse fissure between the caudate lobe superiorly and the quadrate lobe inferiorly, with the hepatic hilum centered on the visceral (inferior) surface. At this gateway you would expect the portal triad, the portal vein positioned posteriorly, the proper hepatic artery anteromedial, and the common hepatic duct anterolateral as they enter or leave the liver. Surrounding landmarks commonly included in a posterior geography study include the groove for the inferior vena cava to the right of the caudate lobe, the ligamentum venosum fissure on the left, and the adjacent peritoneal reflections of the lesser omentum approaching the hilum. Orientation at the porta hepatis matters in hepatobiliary surgery because small errors in identifying anterior versus posterior elements translate into major vascular or biliary injury. This posterior perspective supports teaching of inflow control during a Pringle maneuver at the hepatoduodenal ligament and helps clarify why portal vein injury behaves differently from hepatic artery injury, given its posterior position and thin wall. It also maps directly to oncologic and inflammatory processes at the hilum, including hilar cholangiocarcinoma (Klatskin tumor), where proximal ductal involvement and vascular encasement determine resectability.
