Description
Viewed from an inferior orientation, the right portal fissure is centered as it courses within the visceral surface of the adult male liver toward the porta hepatis. Medial to the fissure lies the hilar plate at the transverse fissure, where the right and left hepatic ducts and the portal vein converge toward the extrahepatic biliary tree. Laterally and posteriorly, the fissure aligns with the right hepatic lobe parenchyma overlying the segmental (Couinaud) anatomy, with the gallbladder fossa situated anteroinferiorly and the inferior vena cava groove positioned more posteromedially. This angle matters because the right portal fissure is a practical road map for inflow and outflow organization, not an academic line on the capsule. During hepatobiliary surgery, surgeons use fissural planes and the inferior hilar view to orient right portal pedicles (Glissonean approach) when planning a right hepatectomy or segmentectomy, and to avoid misidentifying a right anterior sectoral duct as the right hepatic duct. Bile duct injury, ischemic complications from unintended portal branch ligation, and bleeding from short hepatic veins become easier to understand when the portal fissure is seen in relation to the porta hepatis and adjacent segmental boundaries.
