Description
Oriented from an inferior perspective, the medial division of the anatomical right lobe of the liver is isolated, with the visceral (inferior) surface facing the viewer and the diaphragmatic surface directed superiorly. Expect the gallbladder fossa to lie anteroinferiorly on this surface, with the porta hepatis running transversely and separating the caudate process region posteriorly from the quadrate lobe region anteromedially. Medially, the fissure for the ligamentum teres and the groove for the ligamentum venosum define the boundary toward the anatomical left lobe, while the inferior vena cava impression sits posteriorly near the caudate lobe. Clinically, this cut of anatomy maps cleanly onto segmental liver teaching, because the medial division of the right liver corresponds to Couinaud segment V (and adjacent segment VIII superiorly), separated functionally by the course of the middle hepatic vein and portal pedicles rather than by external lobar landmarks.
