Description
Positioned within the right hepatic lobe, the medial division is defined by its relationship to the main portal fissure and the middle hepatic vein, which courses in a cranio-caudal direction toward the inferior vena cava. From a superior perspective, the hepatic dome forms the most cranial contour, while the anterior surface faces toward the diaphragm and the inferior aspect would lead toward the visceral surface and porta hepatis. Medially, this territory approaches the plane that separates right from left functional liver, and laterally it blends into the more peripheral right parenchyma. Segmental boundaries are implied rather than sharply demarcated. Segment-level orientation matters when you plan hepatectomy or interpret cross-sectional imaging, because lesions often respect portal venous territories more than traditional lobar anatomy. A superior view aligns well with operative and radiologic thinking about the hepatic venous drainage, where the middle hepatic vein frequently defines the line of transection between right and left hemilivers and guides parenchymal-sparing resections. Small differences in where you place that plane can determine whether segments V and VIII remain adequately drained or require reconstruction. Clear landmarks.
