Description
Oriented in anatomical position, the superior (diaphragmatic) surface of the left hepatic lobe is presented with attention to the medial division, corresponding to segment IV and often termed the quadrate region when referenced from the visceral aspect. Medially, this territory approaches the fissure for the ligamentum teres and the umbilical portion of the left portal fissure, while laterally it grades into the left lateral segments (II and III) across a subtle surface contour rather than a true external septum. Anteriorly it relates toward the falciform ligament attachment, and posteriorly it rounds toward the area overlying the porta hepatis and the left hepatic vein drainage path. Surface landmarks dominate. This angle matters when you need to reconcile what you see in cross sectional imaging with what you can actually palpate or mobilize in the operating room, because segment IV is functionally defined by portal inflow and hepatic venous outflow rather than by a visible capsular boundary. Segment IV anatomy drives planning for left hepatectomy, extended left hepatectomy, and parenchyma sparing resections, where the middle hepatic vein plane and the relationship to the umbilical fissure determine safe margins. Variation is the rule.
