Description
Centered on the medial segment of the liver, the superior surface is presented with the quadrate lobe region (Couinaud segment IV) emphasized along the medial margin of the left hepatic lobe. The falciform ligament attachment line and its relation to the fissure for the ligamentum teres are oriented in the midsagittal plane, separating the medial segment from the more lateral left hepatic parenchyma. Posteriorly, the superior surface slopes toward the area adjacent to the inferior vena cava and the hepatic venous confluence, while the anterior margin approaches the diaphragmatic dome. Overall contours are clear. Appreciating the topography of the medial segment on the superior surface matters when planning an anatomic left hepatectomy or a segment IV resection, where the surgeon must anticipate the course of the middle hepatic vein and the proximity of the hepatic veins to the suprahepatic IVC. Segment IV is also a common site referenced in staging and operative descriptions for hepatocellular carcinoma and colorectal liver metastases, and its relationship to the falciform ligament can guide intraoperative orientation when surface landmarks are all you have. Small errors here change margins.
