Description
Arising on the posterior aspect of the liver, the caudate lobe (lobus caudatus, Spigelian lobe) occupies a dorsal, medial position beneath the hepatic dome in a superior, cranial-oriented view. Its parenchyma sits posterior to the porta hepatis and just inferior to the diaphragmatic surface, with the inferior vena cava groove immediately posterior and slightly to the right. Medially, the caudate relates to the fissure for the ligamentum venosum, while its rightward continuity approaches the caval region and its leftward margin tapers toward the left hepatic lobe. The impression is of a compact posterior medial segment tucked high under the diaphragm. For surgical anatomy, this is the segment that refuses to behave like the rest of the liver. The caudate has short hepatic venous drainage directly into the inferior vena cava and a dual inflow from right and left portal pedicles, details that drive operative planning during caudate lobectomy, hilar cholangiocarcinoma resection, and transplant graft preparation. A small, cranially hidden lesion here can be missed on cursory assessment, and its proximity to the caval wall and hepatic venous confluence makes exposure and bleeding control a constant concern.
