Description
Seen from a superior profile, the lobus caudatus (Spigelian lobe) occupies the posterosuperior visceral surface of the male liver, tucked between the fissure for the ligamentum venosum on the left and the groove for the inferior vena cava on the right. Its anterior margin blends into the porta hepatis, where the caudate process extends toward the right lobe, bridging to the region of the hepatoduodenal ligament. Posteriorly, the caudate lobe lies immediately anterior to the caval impression, with its surface oriented toward the diaphragmatic dome and the bare area superiorly. This angle matters because the caudate lobe behaves differently from adjacent hepatic parenchyma in both drainage and surgical access, with short hepatic veins entering the inferior vena cava directly and limiting safe mobilization along the posterior liver. Small, deep lesions here challenge both ultrasound and laparoscopic palpation, and segment I hypertrophy in cirrhosis can narrow the retrohepatic caval space and complicate exposure during hepatectomy.
