Description
Viewed from a superior vantage, the male liver’s posterior (visceral) surface comes into relief with the diaphragmatic dome curving anterosuperiorly and the posterior margin sloping inferoposteriorly toward the bare area. The right lobe occupies the lateral and posterior field, while the left lobe tapers medially toward the fissure for the ligamentum venosum, with the caudate lobe positioned posteriorly between the inferior vena cava groove and the fissure. Impressions for adjacent structures are suggested along the inferior aspect, including the renal and colic relations of the right lobe, with the porta hepatis lying anterior to the caudate process and deep to the hepatic hilum. Orientation matters. This superior view is the one you want when teaching or planning around the retrohepatic inferior vena cava and hepatic venous outflow, where short hepatic veins tether the posterior liver and can complicate mobilization. Surgeons working in the right upper quadrant use these posterior landmarks when entering the lesser sac, controlling bleeding near the caval groove, or assessing tumor proximity to hepatic veins in segmental resections. It also supports anatomic correlation with cross sectional imaging, since the bare area, caudate lobe, and caval groove are frequent sources of orientation errors on CT and MRI. Ideal for gross anatomy lab teaching on peritoneal reflections, hepatic segments (with emphasis on the caudate lobe), and operative anatomy modules covering liver mobilization and hepatic venous control.
