Description
Seen from a superior vantage, the anterior diaphragmatic surface of the adult male liver fills the right upper quadrant beneath the right hemidiaphragm, with a smaller portion crossing the midline toward the left. The right lobe lies lateral and slightly posterior to the left lobe, while the convex hepatic parenchyma conforms to the dome of the diaphragm, rising superiorly toward the central tendon. Along the anterosuperior contour, the falciform ligament would be expected to separate the right and left anatomical lobes and course toward the anterior abdominal wall, even when the visceral (inferior) surface is not emphasized. Orientation of the diaphragmatic surface matters when you need to reconcile surface anatomy with operative exposure. During open or laparoscopic hepatectomy, mobilization begins by releasing the falciform and coronary ligaments to drop the liver inferiorly off the diaphragm, and this superior perspective clarifies why subphrenic collections track along the hepatic dome. It also supports teaching around referred shoulder pain via diaphragmatic irritation (phrenic nerve) in subphrenic abscess or postoperative subdiaphragmatic fluid.
