Description
Dominating the right upper quadrant, the liver sits immediately inferior to the right hemidiaphragm, its convex diaphragmatic surface conforming to the inner contour of the lower rib cage. From an anterior perspective, the right lobe occupies a broad lateral expanse beneath ribs 7 to 11, while the smaller left lobe extends medially toward the epigastrium and left costal margin. Costal cartilages and the anterior thoracic wall frame the superior border, and the thoracic vertebral column remains visible posteriorly as a midline skeletal landmark. Only a sliver of pelvis enters inferiorly to anchor orientation. Anterior liver views matter when you need to teach or plan around surface anatomy, because the organ’s superior margin is largely hidden by the costal arches and its inferior edge becomes clinically palpable only when enlarged or displaced. Hepatomegaly from congestive heart failure, acute hepatitis, or infiltrative disease brings the inferior border below the right costal margin, a classic bedside finding that this rib-to-parenchyma relationship clarifies. Needle paths for percutaneous liver biopsy and intercostal approaches for abscess drainage also depend on knowing where hepatic parenchyma sits relative to ribs and pleura.
