Description
Posteriorly, the right hepatic lobe dominates the field, its convex diaphragmatic surface curving superiorly toward the bare area where peritoneum reflects off the liver. The inferior border sweeps anteroinferiorly toward the visceral surface, while the medial margin approaches the fissure for the ligamentum venosum and the plane that separates the right lobe from the caudate lobe. Laterally, the hepatic contour rounds toward the costal impression, and superiorly it relates to the diaphragm, with the retrohepatic segment of the inferior vena cava expected immediately medial and deep to the bare area. Orientation is posterior. Boundaries matter. A posterior view of the right lobe is the one you want when teaching the bare area and the retrohepatic relationships that complicate mobilization, because the peritoneal reflections at the coronary ligament and the right triangular ligament define where blunt dissection becomes avulsion risk. Surgeons encounter this anatomy during right hepatectomy, living donor procurement, and trauma laparotomy, when short hepatic veins entering the IVC from the posterior right lobe can tear and retract. Radiologists also lean on these landmarks when localizing lesions to posterior segments (classically VI and VII) on axial CT or MRI, where the diaphragm and IVC provide fixed reference points.
