Oriented in anterior view, the spleen’s facies diaphragmatica (costal or outer surface) fills the frame as a smooth, convex expanse that faces superiorly and laterally toward the left hemidiaphragm and lower ribs. The organ’s anterior border comes forward toward the left hypochondrium, while its posterior margin turns back toward the paravertebral gutter, helping define the typical oblique long axis along the line of the 10th rib. A subtle transition toward the superior and inferior poles can be appreciated even without emphasizing the visceral surface. Contour matters. Clinically, this anterior presentation of the diaphragmatic surface is the one you mentally reconstruct when correlating physical exam and imaging with splenic size and position. Splenomegaly causes the costal surface to project anteroinferiorly below the left costal margin, a pattern that tracks with percussion and palpation findings and is commonly confirmed on ultrasound or CT. Trauma also targets this surface: blunt left thoracoabdominal injury can produce subcapsular hematoma or laceration along the convex diaphragmatic aspect, with pain sometimes referred to the left shoulder via the phrenic nerve (Kehr sign) when diaphragmatic irritation occurs.
