Description
Viewed from posterior, the diaphragmatic (superior) surface of the liver forms a broad convex dome beneath the right hemidiaphragm, with the left lobe tapering superomedially toward the midline. The bare area lies on the posterosuperior aspect, bounded by the anterior and posterior layers of the coronary ligament that reflect onto the diaphragm, while the right and left triangular ligaments mark the lateral limits of peritoneal reflection. Inferior to this diaphragmatic surface, the rounded posterior border leads toward the visceral (inferior) surface and the posterior groove related to the inferior vena cava. Posterior liver anatomy matters because it frames the hepatic venous outflow tract at the hepatocaval junction and the nonperitoneal bare area that adheres directly to diaphragm. Surgeons rely on these landmarks during mobilization of the right lobe for hepatectomy, trauma packing, or transplantation, where division of the coronary ligament and control of short hepatic veins near the inferior vena cava can be the step that turns straightforward dissection into brisk hemorrhage. Orientation is everything. Use this posterior diaphragmatic surface view in gross anatomy teaching to clarify peritoneal reflections (coronary and triangular ligaments) and to contrast the diaphragmatic facies hepatis with the visceral surface. It also fits well in operative atlases and hepatobiliary surgery presentations when discussing right liver mobilization, retrohepatic inferior vena cava exposure, and subphrenic collections that track along the bare area.
