Description
Viewed in left lateral profile, the epicardial layer (visceral layer of serous pericardium) invests the external surface of the left atrium and left ventricle, continuing into the coronary sulcus and the anterior and posterior interventricular grooves. A thin mesothelial (serosal) lining lies superficial to the subepicardial connective tissue and fat, which cushions and partially obscures the proximal epicardial coronary vessels on the lateral wall. Deep to the epicardium sits the myocardium, with the epicardial reflection approaching the pericardial sac at the roots of the great vessels. Outer coat. Inner fat. This lateral emphasis matters when you need to teach or plan around the plane where pericardial and myocardial pathology meet, because epicardial fat thickness varies along the left ventricular free wall and can mimic or mask disease on imaging and at surgery. Electrophysiology teams also work in this space during epicardial mapping and ablation for ventricular tachycardia, advancing within the pericardial cavity while staying superficial to the myocardium and mindful of the left phrenic nerve course along the fibrous pericardium. Pericarditis and pericardial effusion are understood best when the visceral and parietal serous layers, and their lubricated interface, are clearly separated in the learner’s mind. Use this asset for thoracic anatomy and cardiovascular blocks, for figures in cardiology texts discussing serous pericardium versus fibrous pericardium, and for surgical education on pericardiotomy, epicardial coronary artery exposure, and access for epicardial pacing leads.
