Description
Oriented in left lateral profile, the right coronary leaflet (cuspis dextra, valvula aortae) is seen as a thin semilunar fold projecting into the aortic root, with its free margin directed superiorly toward the sinotubular junction and its attachment curving inferiorly along the aortic annulus. At the midpoint of the free edge sits the nodulus (nodulus valvulae aortae), flanked by the lunulae that taper toward the commissures where this cusp meets the left coronary and noncoronary leaflets. Posterior to the leaflet lies the aortic sinus, and anteriorly the leaflet’s belly relates to the left ventricular outflow tract as it rises from the interventricular septum toward the ascending aorta. A small structure. Big consequences. For teaching valve competence, this lateral view clarifies how the cusp coapts, because the fibrous nodule marks the central contact point that must meet its counterparts to prevent diastolic regurgitation. It also helps explain why small changes in cusp geometry, seen in bicuspid aortic valve phenotypes or after endocarditis-related cusp perforation, can shift the coaptation line and create an eccentric aortic regurgitant jet on echocardiography.
