Description
Oriented in left lateral perspective, the left coronary leaflet (cusp) of the aortic valve appears as a thin semilunar fold with a convex free margin and a basal attachment along the aortic annulus at the left ventricular outflow tract. Superiorly, the cusp faces the aortic root (bulbus aortae), while inferiorly it relates to the left ventricular cavity; its coaptation surface meets the adjacent noncoronary and right coronary cusps at the commissures. Just above the cusp, the left coronary sinus forms a shallow pocket, positioned leftward and slightly posterior in the aortic root. A small but telling landmark. This side-on view matters when you need to teach or plan around the geometry of valve closure, since aortic regurgitation often results from cusp prolapse, fenestrations along the lunula, or annular dilation that prevents full coaptation. Surgeons and interventionalists rely on the relationship between the left coronary cusp and the left coronary ostium in the corresponding sinus during valve-sparing root repair, aortic root replacement, and transcatheter aortic valve procedures where coronary obstruction is a feared complication. Spatial orientation is the point.
