Description
Oriented in left lateral profile, the aortic root is presented at the junction of the left ventricular outflow tract and the ascending aorta, with the three semilunar cusps of the valva aortae forming a crown-like leaflet complex. Each cusp arises from its corresponding sinus of Valsalva, and their free margins meet centrally at the commissures, creating the coaptation line that lies obliquely to the long axis of the aorta. From this side view, you can appreciate the fibrous continuity between the aortic annulus and the ventricular myocardium inferiorly, while the tubular ascending aorta extends superiorly and slightly posteriorly relative to the left ventricle. The cusp bodies sit anterior and posterior to one another depending on the leaflet, with the sinotubular junction defining the superior boundary of cusp excursion. A left lateral view matters when you need to teach or plan around outflow-tract geometry rather than the en face “surgeon’s view.” Cusp restriction in calcific aortic stenosis often stiffens the belly of the leaflet and reduces systolic opening at the sinotubular junction, while aortic regurgitation frequently relates to annular dilation or sinotubular junction enlargement that prevents proper leaflet coaptation. TAVR sizing and surgical valve-sparing root procedures both rely on understanding the relationship between annulus, sinuses, and sinotubular junction. Small structures. Big consequences.
