Description
Centered in a transverse section through the cardiac base, the aortic root is opened to expose the left coronary leaflet (left coronary cusp) of the valvula semilunaris aortae, its crescent free margin projecting into the aortic lumen. The cusp attaches inferiorly along the annulus at the left aortic sinus, positioned anterior to the left atrium and superior to the left ventricular outflow tract. Adjacent structures typically included at this level are the other semilunar cusps, the sinotubular junction, and the proximal ascending aorta. Orientation is clean. Emphasis on the left coronary leaflet matters because it defines the left aortic sinus, the site of the left coronary ostium and the origin of the left main coronary artery, a relationship that drives both pathology and operative strategy. Endocarditis vegetations, cusp prolapse, or calcific aortic stenosis frequently distort cusp coaptation and can obstruct or compromise coronary flow if disease extends into the sinus.
