Description

Centered in a frontal perineal view, the external male anal opening is shown as the anal orifice (anus) bordered by the anal verge and surrounding perianal skin. Radially oriented cutaneous folds converge toward the lumen, while the pigment transition at the mucocutaneous junction defines the margin between the anal canal lining and keratinized integument. Superiorly (anterior in anatomical position) the perineal skin extends toward the root of the scrotum, and inferiorly (posterior) it continues into the intergluteal cleft toward the coccygeal region. Orientation at the anal verge matters because it is where dermatologic and colorectal disease intersect. External hemorrhoids arise from the subcutaneous external hemorrhoidal plexus distal to the dentate line and present at or just outside this opening, while anal fissures often occur in the posterior midline and are evaluated by careful inspection of the verge and adjacent skin. Local anesthesia, incision and drainage of a perianal abscess, and placement of setons for fistula-in-ano all begin with accurate identification of the external orifice and the surrounding perianal landmarks. Small distances here carry consequences. Suitable for undergraduate gross anatomy, pelvic floor and colorectal modules, and clinical skills teaching on anorectal examination and common perianal pathology in adult men.
