Description
Anterior sectioning through the adult female urinary bladder exposes the detrusor vesicae with emphasis on its external longitudinal smooth muscle layer. Longitudinal bundles course superior to inferior along the bladder wall, lying superficial to the deeper circular and inner longitudinal components and deep to the serosa or adventitia depending on peritoneal coverage. The bladder lumen sits centrally, with the trigone and bladder neck positioned inferoposteriorly relative to the dome, where the muscular wall typically appears thicker. Orientation of the external longitudinal detrusor matters when you teach or plan around bladder outlet mechanics and iatrogenic injury. During anterior pelvic surgery, including hysterectomy and bladder neck procedures for stress urinary incontinence, detrusor fibers near the vesicourethral junction and anterior bladder wall can be inadvertently entered, predisposing to cystotomy or postoperative detrusor overactivity. Fiber direction also frames the discussion of why detrusor contraction propagates as a coordinated squeeze rather than a focal pinch, and why overdistension can blunt contractility in acute urinary retention.
