Description
A fetus of approximately 15 weeks gestation lies in a flexed attitude within the amniotic cavity, enclosed by the translucent amniotic membrane and surrounded by amniotic fluid. The head remains proportionally large, with a smooth domed calvaria, closed eyelids, formed external ear and a recognisable facial profile; the upper limb is drawn toward the chest with the fingers of the hand separated, while the lower limbs are flexed at hip and knee. The umbilical cord inserts at the anterior abdominal wall and spirals upward, its coiled umbilical vein and paired umbilical arteries visible through the sheath, to reach the discoid placenta seen at upper right with its branching chorionic vessels. The vascular endometrial and myometrial layers of the uterine wall frame the gestational sac. By the mid-second trimester the fetus has completed organogenesis and is in a phase of rapid growth, and this is the window in which routine obstetric ultrasound assesses biometry, limb and cranial morphology, placental site and cord insertion. The depicted relationship between cord, placenta and uterine wall underpins discussion of placental localisation, marginal or velamentous cord insertion, and single umbilical artery. The image also provides the anatomical context for second-trimester procedures such as amniocentesis, and for counselling on fetal movement, swallowing of amniotic fluid and fluid dynamics.
