Description
At approximately 23 weeks of gestation the fetus lies curled in a flexed attitude within the amniotic cavity, seen here in left profile with the head uppermost and the limbs drawn toward the trunk. The rounded cranium, low-set ear with a formed helix, closed eyelids and slightly parted lips are clearly modelled, while the hands are held together in front of the chest and the legs cross at the ankles with distinct toes and heels. The umbilical cord emerges from the abdominal wall and coils toward the vascular placental surface visible as a broad plate on the far side of the sac. Enclosing everything, the translucent amniotic membrane and the surrounding amniotic fluid separate the fetus from the richly perfused uterine wall, which frames the composition in deep red. The middle of the second trimester is when detailed anatomical survey and growth assessment are performed, and this is the period in which viability begins to be discussed with parents. Fetal position, cord insertion and placental location are all part of routine ultrasound reporting, and the relationship shown here between cord, placenta and amniotic cavity underlies assessments of cord coiling, amniotic fluid volume and placental site. Skin at this stage remains thin and translucent with subcutaneous fat only beginning to accumulate, and the coordinated hand-to-face and lower limb movements depicted correspond to the fetal activity mothers commonly report as quickening.
