Description
A fetus at approximately 19 weeks of gestation lies in a flexed attitude within the translucent amniotic sac, shown in left lateral profile against the vascular uterine wall. The head is disproportionately large relative to the trunk, with the eyelids fused, the nose and lips modelled, and the external ear now clearly formed on the side of the head. The upper limbs are flexed at the elbows with the hands brought toward the chest and the fingers separated, while the lower limbs are drawn up in front of the abdomen with the plantar surfaces of the feet visible. The umbilical cord emerges from the abdominal wall and loops back through the amniotic fluid toward the discoid placenta, its coiled umbilical vein and paired umbilical arteries distinguishable through the cord surface. The mid-second trimester is the window in which most detailed obstetric ultrasound assessment is performed, making this stage central to biometry of the biparietal diameter, head and abdominal circumference and femur length, as well as to survey of the limbs, face and cord insertion.
