Description
A fetus of approximately 26 weeks gestational age lies in a flexed attitude within the intact amniotic sac, seen from the left side with the head uppermost and the limbs drawn toward the trunk. The fetal head is disproportionately large relative to the trunk, with closed eyelids, formed external ear and fine scalp hair; the hands are held close to the face and the lower limbs are flexed at hip and knee, with well-defined digits on the hands and feet. The umbilical cord passes from the anterior abdominal wall toward the placenta, where branching chorionic vessels spread across the chorionic plate on the posterior uterine wall. Translucent amniotic membranes and surrounding amniotic fluid separate the fetus from the myometrium, whose vascularised inner surface frames the gestational compartment. At 26 weeks the fetus is well into the late second trimester, with subcutaneous fat still sparse, thin skin and rapidly maturing lungs entering the saccular stage of development; this is the period during which surfactant production and neurological maturation determine outcomes for extremely preterm birth. Views of this stage support discussion of viability thresholds, antenatal corticosteroid timing, fetal growth assessment by biometry, amniotic fluid volume evaluation and cord and placental localisation on ultrasound. The depicted relationship between cord insertion, placental site and uterine wall is also relevant to counselling on placental position, cord accidents and fetal presentation.
