Description
The fetus lies in a flexed attitude within the amniotic cavity, viewed from the left side so that the rounded cranium, the auricle of the ear, the closed eyelids and the curve of the vertebral column are all visible in one profile. The upper limb is drawn up toward the face with the fingers near the mouth, while the lower limbs are folded so that the plantar surface of one foot with its individually formed toes lies against the trunk. The umbilical cord emerges from the abdominal wall and spirals across the field, its coiled vessels distinguishable, inserting into the chorionic plate of the placenta that clings to the uterine wall above. Diffuse red tones of the decidua and myometrium enclose the whole scene. At twenty weeks the fetus is at roughly the midpoint of gestation, the stage of the routine mid-trimester anomaly scan. This is when sonographers assess biometry, cranial contour, spine, limb integrity, cord insertion and placental site, and when placental position relative to the internal cervical os is documented. The relationship between cord, placenta and uterine wall shown here underlies discussion of cord coiling, cord entanglement and velamentous insertion, while the visible external ear, digits and plantar creases correspond to the surface features clinicians and parents recognise from ultrasound imaging at this age.
