Description
A fetus at approximately 23 weeks of gestation lies in a flexed attitude within the intact amniotic sac, seen in profile from the left side. The head is disproportionately large relative to the trunk, with a smooth prominent forehead, closed fused eyelids beginning to separate, a well-modelled auricle standing away from the scalp, and a slightly open mouth. The upper limbs are drawn toward the chest with the fingers of both hands touching, while the lower limbs cross at the ankles in the classic fetal posture. The umbilical cord emerges from the abdomen and spirals toward the chorionic plate of the placenta, whose vascular surface is visible through the translucent membranes; the endometrium and thickened uterine wall enclose the amniotic fluid space, and the skin is thin, wrinkled and vascular with vernix-poor translucency typical of this stage. The twenty-third week sits at the threshold of extrauterine viability, when pulmonary development advances from the canalicular into the saccular phase and surfactant production begins in earnest, making gestational dating and growth assessment clinically decisive.
