Description
The fetus lies in a flexed attitude within the translucent amniotic sac, seen from the left side with the head uppermost and the vertex resting against the sac membrane. Surface features expected at this stage are clearly modelled: a well formed auricle standing away from the head, closed and fused eyelids beneath prominent brows, a slightly parted mouth, and thin translucent skin over which the subcutaneous fat layer is still sparse. The hands are brought together in front of the chest with individualised fingers, while the lower limbs are crossed at the knees with the soles and toes turned toward the viewer. The umbilical cord emerges from the abdominal wall and coils toward the vascular placental surface behind the fetus, with amniotic fluid filling the sac and the muscular uterine wall forming the surrounding chamber. Twenty-three weeks sits at the threshold of extrauterine viability, so this stage carries particular weight in obstetric counselling and neonatal planning. It corresponds to the period of rapid alveolar and surfactant development, active swallowing and fetal breathing movements, and responsiveness to sound, and it is the window in which anatomy surveys, growth biometry and amniotic fluid assessment guide decisions about preterm care. The intact membranes and cord insertion depicted here are also the reference anatomy for discussions of preterm premature rupture of membranes, cord position and placental function.
