Description
At approximately 36 weeks of gestation the fetus fills most of the uterine cavity, shown here in the characteristic flexed attitude with the spine curved, the chin tucked toward the chest and the limbs drawn in against the trunk. The head is directed toward the lower left of the frame, scalp hair well established, with the external ear, closed eyelids and rounded facial contours clearly modelled. The fingers of one hand rest near the face while the knees and feet are folded upward, individual toes distinguishable. The translucent amniotic membrane encloses the fetus in a reduced volume of amniotic fluid, the umbilical cord loops between the limbs, and the vascular uterine wall with the darker placental surface forms the surrounding chamber. Near term, the fetus has usually settled into a longitudinal lie, and the relationship between fetal poles, amniotic fluid volume and uterine wall is central to obstetric assessment. This is the anatomy behind Leopold manoeuvres, fundal height measurement, ultrasound biometry and biophysical profile scoring, and behind decisions about presentation, engagement and mode of delivery. Late third-trimester findings such as oligohydramnios, breech or transverse lie, cord entanglement and reduced fetal movement are all interpreted against this baseline of a healthy, well-grown fetus with normal flexion and adequate fluid.
