Description
A fetus of approximately 36 weeks gestation lies curled in a flexed attitude inside the intact amniotic sac, surrounded by amniotic fluid and enclosed by the vascularised uterine wall. The head is well developed with fine scalp hair, closed eyelids, a formed external ear and rounded facial contours, and the limbs are drawn tightly against the thorax and abdomen in the characteristic fetal position typical of the crowded third-trimester uterine cavity. Fingers and toes are fully separated with visible nails, and subcutaneous fat has smoothed the skin surface. The umbilical cord, with its coiled vessels indicated in blue and purple, passes from the abdominal insertion toward the discoid placenta seen behind the fetus against the uterine lining. Near term the fetus has usually settled into a longitudinal lie with the vertex presenting, and this depiction illustrates the spatial constraints that determine presentation, engagement and the assessment of fundal height. Lung maturation, weight gain and accumulation of brown adipose tissue during weeks 34 to 37 underpin the clinical distinction between late preterm and full-term birth. The visible relationships between cord, placenta and uterine wall are the anatomy behind obstetric concerns such as cord compression, placental position and reduced amniotic fluid volume, all routinely evaluated by third-trimester ultrasound and cardiotocography.
