Description
At approximately 39 weeks of gestation the fetus fills the uterine cavity almost completely, shown here in a flexed attitude with the spine curved, thighs drawn onto the abdomen and one hand raised to the head. The scalp carries visible hair, subcutaneous fat has smoothed the contours of the cheeks, trunk and limbs, and the fingers, toes and external ear are fully formed. The translucent amniotic sac membrane surrounds the fetus, with a reduced volume of amniotic fluid separating skin from membrane. The umbilical cord passes from the abdominal insertion toward the placental disc at the right, its chorionic vessels branching across the fetal surface of the placenta, which lies apposed to the endometrium and myometrium of the uterine wall. Week 39 is early term to full term, the point at which pulmonary surfactant production, thermoregulation and suck-swallow coordination are mature enough for extrauterine life. This view illustrates the anatomical basis of clinical assessment at term: fetal lie and presentation determined on Leopold manoeuvres, engagement of the presenting part, cord position relevant to nuchal cord and cord compression, and placental location relevant to placenta praevia and abruption. Amniotic fluid volume, evaluated as the amniotic fluid index, declines toward term, and the relationship between fetal size and uterine capacity underlies discussion of induction, cephalopelvic disproportion and monitoring in the days before labour.
