Description
Arising from the left superior lobar (upper lobe) bronchus, the lingular bronchus courses anteroinferiorly toward the lingula and gives rise to the inferior lingular segmental bronchus (B5). From an anterior perspective, B5 is seen descending inferior to the superior lingular segmental bronchus (B4) and anterior to the more posterior upper lobe segmental branches, tracking toward the anteromedial parenchyma adjacent to the cardiac notch. Proximal airway caliber and branching angles are emphasized. Orientation is left-sided throughout. For bronchoscopy and endobronchial navigation, the inferior lingular segment is a frequent “miss” when secretions pool in dependent airways and the lingula behaves like a functional analog of the right middle lobe. Mucus plugging, aspiration-related atelectasis, and postinfectious bronchiectasis often localize to B4 and B5, and accurate segmental identification guides targeted suctioning, lavage, and bronchoscopic sampling. Segmental anatomy also matters in parenchyma-sparing surgery, since a lingular segmentectomy depends on correct recognition of B5 relative to the lingular artery and vein and to the more posterior upper lobe bronchi.
